From booking to discharge, a clear, step-by-step path

Step 1

Book an Appointment

Start by booking your Appointment to choose a time that fits your schedule. You’ll be asked to upload your ID and insurance details to get started.

  • Click Book Now
  • Choose a timeslot
  • E-consents & crisis info sent automatically
  • Staff confirm & help set up your portal
Step 2

In-take Forms

Once appointment confirmed:  

• Core questionnaires emailed 
• Complete before Session 1

Step 3

Assessment & Counseling

  •  8 Skill-Focused Sessions
  • Weekly video visits (S1–S8)
  •  Study, sleep & executive-function skills
  • After Session 2 → referral email/instructions sent
Step 4

Physician Referral

• Ask your family doctor or a walk-in to refer to CALM
• Important step to continue with Medical management
Step 5

Medical Consults

  • Day 1: review intake, labs/ECG orders, home BP
  • Day 2: full physical exam
  • Day 3: Review results & medication initiation (if appropriate)
  • Day 4: dose review & counselling plan
